Individual Player Application Form
Deadline:
Applications for individual players must be received by the Foundation on or before May 31st, of the current year.
Eligibility:
- Applicants must complete in full the online information form below.
- Upload an Athletes personal statement for funding.
A personal statement written in the athletes own words describing their use for funds, their personal goals and desire to excel at the game of lacrosse.
(Upload formats accepted include Microsoft .doc and Acrobat .pdf formats.)
